Ohio State Sales Tax Exemption form
To receive the exemption, download and fill out the following word document, print it out and take it to the vendor you are purchasing your computer hardware from. If you are ordering online, tell the vendor you are a teacher and exempt from sales tax, and ask what the procedure is for filing your tax exempt form.
